2. Booking a collection

Download Contract for Cremation Form and fax the completed form to Pet Memories on 9890 5911, or phone our toll free 1800 227 784 to arrange pickup with the completed form.

3. Payment

Payment needs to be arranged at the time of pick up. We will provide you with a detailed invoice upon return of the ashes. Accepted payments methods include:

• Credit card
• Direct payment into our account
• Money order
• Cash
• Sorry no cheques
